Appointment requests
Submitting a form is a request, not a confirmed appointment. We confirm every appointment by phone or email. Please don’t use forms for urgent needs; call 911 in an emergency.
Cancellations
Please give us at least 24 hours’ notice to cancel or reschedule. Late cancellations and missed appointments may be charged a $25 fee, which we waive for illness and emergencies.
Prices and payment
Prices listed on this website are in U.S. dollars and may change. The price that applies is the one in effect on the date of your visit. Insurance coverage depends on your plan; we’ll check benefits when asked but can’t guarantee what your insurer will pay.
